It’s currently the fifth largest provider of health insurance in the U.S. Humana serves nearly 9 million Medicare members, over 5 million of whom are enrolled in a Medicare Advantage plan .
According to a Humana customer service representative, its networks are regionally based. Each plan has its own set of providers, specialists and pharmacists.
Plan options vary based on location, but can include health maintenance organization (HMO) plans, preferred provider organization (PPO) plans, special needs plans (SNPs) and private fee-for-service (PFFS) plans.
Private fee-for-service (PFFS) plans essentially allow you to work with any provider who is Medicare-approved and agrees to the conditions of your plan.
Humana HMO plans allow you to choose a doctor from the Humana provider network. With Humana, you’re also not locked into a provider and can switch as you see fit. Most Humana HMO plans include prescription coverage. They also offer hearing benefits, as well as vision and dental benefits, none of which are covered by traditional Medicare.
PPO plans are more flexible in terms of picking providers, so if you have several specialists or a preferred physician, a Humana PPO plan may be best for you.

There are two specific types of SNP plans with Humana: Chronic Condition Special Needs Plans and Dual-Eligible Special Needs Plans. Currently, these plans are available in just under 30 U.S. states.
Plans available to you largely depend on your ZIP code. Plan availability varies widely based on location. For example, in the Boston area (ZIP code 02108), Humana offers seven Medicare Advantage plans, whereas in Hillsborough County, Florida, (ZIP code 33647) the insurer offers 16 Medicare Advantage plans. Meanwhile, some ZIP codes don’t have access to any Humana Medicare Advantage plans.
Most larger states are broken up into four or five sections in which Humana works to contract with as many providers as it can. On Humana’s website, you can shop for plans based on providers who are considered “in network.”
